If you do not have insurance, or if we do not take your particular insurance, we also accept self-pay.

We are happy to assist new and established clients with verifying insurance coverage and benefits. While we make every effort to provide accurate information, coverage and fees may vary based on your specific plan, eligibility, and benefits.

Please note that we may not know your exact out-of-pocket cost until claims are submitted and an Explanation of Benefits (EOB) is received from your insurance company. Ultimately, clients are responsible for understanding their own insurance coverage and benefits.

If you have already emailed your insurance card or submitted this information through your client portal, there is no need to complete the form again.

PAYMENT

Payment is processed through the client portal. We accept cash, check, and most major credit cards.
